Individual community living support (ICLS) is designed for older adults on the Elderly Waiver who want to keep living in their own home, with just enough support to make that safe and sustainable.
What it looks like
- Friendly, scheduled check-ins that family can count on.
- Help coordinating appointments, rides, and household tasks.
- Support with routines: meals, medication reminders, and light activity.
- An early-warning system: staff who notice changes and tell the family.
Who it's for
Seniors who don't need nursing care but shouldn't be navigating every day alone. It's also for families who want reliable eyes on Mom or Dad between their own visits.
How it's paid for
ICLS is an Elderly Waiver service, usually covered at no cost to the family. It starts with a free MnCHOICES assessment through the county. Call or text us and we'll help you find the right contact.
