What it looks like
- Coaching sessions with the family alongside the person served.
- Shared strategies: the same prompts, routines, and expectations from everyone.
- Help navigating disability-related challenges as a household, not alone.
- Goals and progress the case manager can see in writing.
Who it's for
Families who want to be part of the plan, especially households where a young person is building independence and everyone wants to row in the same direction.
How it's paid for
Covered through Minnesota's waiver programs (CADI, DD, BI, and CAC) at no cost to the family, with hours authorized by the county case manager.
