Individualized home supports with training

Hands-on teaching toward independence, with clear, measurable goals the whole team can see.

This is IHS with a mission: instead of doing tasks for the person, staff teach the person to do them: cooking, budgeting, riding transit, health and safety routines, one skill at a time, with progress everyone can measure.

What it looks like

  • Written, person-centered goals chosen with the individual and their team.
  • Hands-on practice: cooking real meals, making real budgets, taking real bus routes.
  • Health and safety skills: medications, appointments, emergencies.
  • Progress notes and regular reviews so the case manager sees movement, not vagueness.

Who it's for

Teens and adults with disabilities working toward more independence: a first apartment, more say in their own day, or skills that make both possible.

How it's paid for

IHS with training is covered through Minnesota's waivers (CADI, DD, BI, and CAC) at no cost to the family, authorized by the county case manager.
